How to Apply for the Ministry of National Food

- Note: An appointment letter is not required.
- Two pictures with a white background
- Original passport with one copy (in color or black and white). Make sure the copy fills a whole A4 page.
- Page Two of your Passport: If you have not already, sign the second page and take a picture of it.
- JW 201/202 Visa Form (one photocopy and the original). You can obtain your JW Visa Form from this website if you have been accepted to a Chinese university.
- The original admissions notice. If the admissions notice is sent by mail, print the scanned copy and turn it in. If it is received in hard copy, create a photocopy of the original.
- Certificate of Police Clearance (original certified by MOFA). If demanded, turn in the original document; if not, turn in a photocopy. You do not require a Police Clearance Certificate if you have visited China within the last five years.
- Degree or Provisional Certificate (approved by MOFA + HEC for a bachelor's degree and beyond). Send in a copy of your degree or provisional certification. Deliver the original paperwork to Gerry's office. It is not necessary to have your prior degree verified by MOFA & HEC if it was earned in China.
- Transcripts (approved by HEC and MOFA for degrees beyond a bachelor's). Provide a copy of your transcripts. Deliver the original paperwork to Gerry's office. - Certificates of Metric and Intermediate Level (Sand, IBCC approved). Send in a photocopy of your original Metric and Intermediate school records. Deliver the original documentation to Gerry's office.
- Certificate of Family Registration (FRC). The NADRA Office can provide you with a Family Registration Certificate if you are using a blood relative's bank statement. Make sure MOFA verifies the FRC.
- Affidavit: You must provide an affidavit attesting to the relationship between you and any blood relative whose bank statement you are using. There is no need to verify from MOFA.
- Details about the Focal Person: On any blank page, enter the name, email address, and phone number of your focal person—your professor who sent the acceptance letter, or your advisor who works with overseas students.
